Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your insurance to verify how your plan compensates you for pain management services, physical therapy, and mental health benefits.
Here are some sample questions to ask your insurance provider to help determine your benefits:
- Do I have a deductible? If so, what is it and have I met it yet?
- Do I need written approval from my primary care physician in order for services to be covered?
- Does my health insurance plan include mental health benefits?
- Does my plan limit how many physical therapy sessions per calendar year I can have? If so, what is the limit?
Rates
For self pay rates, please see our current self pay fee schedule under “Get Started > New Patient Forms.” For insurance eligibility and benefits, please contact the number on the back of your insurance card.
Payments
We accept cash and most major credit cards as forms of payment. Please call our office 303.469.3182 x741 to make a payment over the phone. Payments may also be made on the patient portal by clicking the above button “Patient Portal.”
Cancellation Policy
If you are unable to attend your appointment, please make sure you cancel at least 48 business hours beforehand. Otherwise, you may be charged a cancellation fee.
