Before you can visit your provider via telehealth, you must sign the Telehealth Consent Form under “Get Started > New Patient Forms.”
Be sure to join the waiting room of your provider at least 15 minutes prior to your appointment by clicking on your provider link below. If you need more assistance, view additional instructions under “Our Services > Telehealth.”
